The facility director was provided a copy of the Notice of Inspection Rights. Three of three Fingerprint Clearance Cards reviewed were valid through the DPS website. Please submit a current Violation-Free Fire Inspection Report. The following was discussed, but not limited to: *Rosters are updated as children come in. *Ensure postings are accurate and updated.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 2/21/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was emailed at the time of the inspection. The DES Contact form was completed at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: Ensure children are supervised at all times. Ensure first aid has all required items.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 02/29/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *The room temperatures are to be maintained between 68F and 82F degrees. *The first-aid kits are to be inaccessible to the enrolled children. *The sign in and out sheets are to be completed with full parent signatures. *Ensure that children have identification on field trips. *Ensure that high school staff have current school schedule in the staff file.
