Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Description: No physical punishment

Noncompliance Area: Video footage from 10/28/2025 at 2:17 pm, shows child #1 was having behavioral issues and was running around the room in the preschool classroom during naptime. Staff person #3 was on the phone with the child's parents while staff person #2 was redirecting the child and then had the child sitting in their lap to help them calm down. Staff person #1 walked into the classroom and grabbed child #1 by their left upper arm and lifted the child into the air off of staff person #2s lap. While child is still in the air staff person #1 grabs them by their right arm and releases their left arm and puts them down on their feet. Staff person #1 then leads the child by their right wrist out of the side door of the classroom.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Ratios in a mixed group is determined by the youngest child in care. The staff:child ratio for a young toddler is 1:10 while children are napping. *Corrected at the time of the inspection.

Description: General Health and Safety/Infant sleep position

Noncompliance Area: Observed compact cribs in the infant classroom with full size crib sheets on the mattresses in lieu of crib sheets intended for a compact crib mattress. The crib sheets, which were too big for the mattress, created access material which creates a potential suffocation hazard.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Observed staff person #1 and #2 were unable to identify the children in their assigned groups in the PreK classroom. Staff person #1 initially named 7 children in their group and staff person #2 named 9 in their group. There was a total of 17 preschool children in the classroom at the time. Child #1 was not assigned to a group and was not named by either staff person.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Observed staff person #4 working alone in the infant classroom without complete provisional hire documentation at the time of initial hire (see LIS code sheet). Noncompliance Area: On 9/4/2020, Facility Person #1 left a spray bottle with sanitizing solution on the floor in reach of Child #1. Child #1 grabbed the bottle and continued to mouth the spout of the bottle until a parent pointed it out to the staff.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: It was observed through the facility's video that Facility Person #1 used physical punishment to redirect Child #1 in the classroom. Facility Person #1 picked the child up by the arms and physically moved them across the room in an aggressive manner leaving finger marks and bruises on the child.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: It was observed that Child #1 did not have a completed incident report for an incident occurring at the facility on June 29, 2020.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: It was observed that the facility director and owner did not immediately report suspected mistreatment of Child #1 to Child Line. The incident occurred on 6/29/202, incident was reported on 7/02/2020.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law.

Noncompliance Area: Outdoor play climber has significant crack in the plastic surface on the slides launching area. Could pinch or cause harm to children while at play.

Noncompliance Area: Imbedded gross motor climbing equipment did not have the recomended protective covering (Mulch) amounts as recommended by the USCPSC.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission.
