Provider Response: (Contact the State Licensing Office for more information.) Consent forms are completed on ILead during Orientation. Paper consent forms and FI fingerprints were on file at the time of the inspection. Documented ongoing issues with ILead dating back to 2019. Missing staff have submitted consent forms to ILead again. I have also attached where this is an ongoing issue with ILead dating back to 2019. I sent you the email titled "I hate ILead" on Feb. 14th 2020 and Evonne responded that is was a 'common problem they are working on". I also attached the email from Chris at ILead acknowledging the problem on the same date. On March 30th, I sent Chris another email that showed I was still having the same issues and that i had been kicked off ILead. the website showed that MY birthdate was now wrong and MY consent form stated I was Brittany C. I followed up again on June 19, 2020, because Amber had been connected between 8-10 different times. Once was by their representative during a login session that only lasted 24 hours before she got kicked out again. I have also attached the emails where I have had to re-invite myself on 4 different occasions because I was being deleted from m own facility. I did the original but then had to redo it on Dec. 2, 2019, March 30th (because they had connected Brittany C. in my placed) and again on May 11, 2020. I am attaching the hard copies for both Melia and Avery that were available at the time of the inspection. |
