A safe indoor and outdoor environment was not provided for the children. On November 17, 2022, staff members failed to provide a safe environment for a three-year-old child. The child was able to push a broken gate open and leave the playground, placing the child at risk of harm. In addition, though a request by administration had been made for the gate to be fixed weeks prior to the incident occurring, children were still allowed to play in the area prior to the gate being fixed. |
