On 6/5/24 A child had a seizure, due to a fever, and was taken to the hospital by the ambulance, and no incident report was submitted to DCDEE within 7 days. Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence height in multiple areas was below 4 feet tall.

Children were not adequately supervised at all times. A child ran from the teacher during a transition and was able to get the door going outside open and a few steps outside. The child was in the sight of the Assistant Director before he was able to open the door.
