| Findings: There is one child that is missing 2 DTAP and 1 Polio immunization. The parent picked this child up while the licensing specialist was conducting the inspection. The issue was discussed with the parent and she stated she thought the child was up to date on all immunizations. She will contact the doctor's office and obtain an updated immunization record or a note from the doctor if she is on a delayed schedule. Another child did not have any Hep B immunizations and the immunization record stated refused. The provider called the parent during the inspection and the licensing specialist spoke to her; she said that she did refuse but thought the child eventually received the immunizations. The parent will look through her records or contact the doctor's office and obtain an updated immunization record.
