Findings: In review of the children's files that were observed at today's inspection, there is missing documentation that was not located in the children's files. In need of Health Inventory Part I and II, immunizations and the lead screening form. The Licensing Specialist gave a list of hand-written notations to the provider of the forms that are missing from the children's files. Obtain these forms and letter of correction to the Specialist within 30 days of the date of this inspection.

Findings: The Licensing Specialist was informed at the time of the inspection that the provider's daughter and her son-in-law moved into her residence in January 2021. The daughter was present and stated she and her spouse are moving out of this residence at the end of January 2022. The provider was informed that OCC is required to be notified of a change in residents. The required paperwork will be mailed to the provider in order for the clearances to be conducted. The provider has not cared for child care children in her home for several years.

Findings: The provider's CPR and First Aid has expired. The provider stated she has taken the course; however, she could not locate her certificate. Please submit a copy of the certifications to the Licensing Specialist. If the certification can not be located, the course will need to be retaken.

Findings: Please submit at least 6 additional hours of continued training by 2/28/2020. The provider did not complete the required amount of continued training within the required timeframe.
