Findings: Staffing patterns that were posted and what licensing has, do not match what is currently going on in the rooms. Room 1 is noted as being the preschool room and room 3 is noted as being the school age room. There were staff who were not working where they are showing on the staffing patterns. New staffing patterns for the entire center need to be posted and also sent to licensing. Correct immediately.

Findings: Carol Lomax was observed with a group of children including toddlers, preschoolers, and school age children in room 1. This is not appropriate grouping of children, therefore, supervision is not in compliance. Further, Carol Lomax was observed in and out of the room on several occasions, to address staffing issues and receiving children as they entered the center, leaving children in room 1 unsupervised. This is a repeat violation that was observed at the last inspection. Further, 5 infants and toddlers mixed with 6 preschool age children, was observed in room 3, and Breswia Eshaun was leaving the room with no other staff, is a clear violation of supervision. By 9:45 am, this was corrected. Additionally, licensing observed at least 3 preschool children walking in the hallway, unsupervised. They were going and coming from one room to another, and going and coming from the bathroom. Correct immediately.

Findings: There was not enough staff upon licensing specialists arrival. Breswia Eshaun was working alone in room 3. She would leave the 12 children alone to answer the door to check in children and conduct thermal scans. We observed Breswia Eshaun away from the class for approximately 10 minutes, checking in more than 5 children who arrived back to back. This group size is not permitted with the 5 infants/toddlers that were in the room and mixed with preschoolers. Carol Lomax was in room 1 with toddlers, preschool children and school age children. There is no ratio for this, as this grouping is not permitted, Whitney Ross was observed in room 5, with 9-2 year olds, which requires 2 staff. This was corrected by 9:45am.

Findings: In room 4, a Breswia Eshaun took a child to the bathroom, leaving Cheila Boyd alone with 5 toddlers. At no time can 1 staff member have more than 3 toddlers.

Brilliant Beginnings is an EXCELLENT facility. My granddaughter has been attending since she was 1 year old now 4. The staff are motiviated and dedicated to the children they are assigned to. Each class is broken down by age which makes the learning experience special and more effective. Thanks to BB my granddaughter started in Pre-K and will be skipped to the 1st grade in Sept. THANKS BB.
