At the visit conducted on October 16, 2019, nine infants were present with one staff member. The proper staff-to-child ratio for infants is one qualified staff member for every four children. The maximum group size for infants is eight children. On a follow up visit, an unqualified staff was observed alone with five infants. Ms. Girgis was observed outside of her assigned classroom on multiple occasions. To count in staff-to-child ratio, a person must stay inside the classroom or area with the children. The attendance times were also different than the times on the sheet LS Flowers documented at the visit. The staff must use ink when recording attendance because OCCL has received multiple reports of altered records and has observed altered records. Records are not to be altered.
