Corrective Action: Ensure proper staff-to-child ratios and maximum group size are maintained at all times. On October 16, 2019, the three-year-old classroom staff member left the classroom to answer the door leaving the classroom unsupervised. At the visit conducted on October 16, 2019, nine infants were present with one staff member. The proper staff-to-child ratio for infants is one qualified staff member for every four children. The maximum group size for infants is eight children. On a follow up visit, an unqualified staff was observed alone with five infants. Ms. Girgis was observed outside of her assigned classroom on multiple occasions. To count in staff-to-child ratio, a person must stay inside the classroom or area with the children. Infants and toddlers were combined with older children, although it was not the first or last 90 minutes of the center’s hours of operation. Ensure infants and toddlers are separated after the first and last 90 minutes of the center’s hours of operation. Ms. Girgis must complete the following two hours of quality-assured training, which can be found on the Better Kid Care website: Balancing Child-Staff Ratios. Submit documentation of completion to OCCL by November 20, 2019.

Corrective Action: Ensure files and records applicable to licensing are accurate. Ms. Girgis submitted to OCCL copies of the infant classroom attendance that were altered. Based on a picture taken by the specialist during the visit, the attendance sheet that Ms. Girgis submitted showed the name of the fifth child signed into the infant room on October 17, 2019, was different than the name of the child who was signed in on the sheet at the time of the visit. The attendance times were also different than the times on the sheet LS Flowers documented at the visit. The staff must use ink when recording attendance because OCCL has received multiple reports of altered records and has observed altered records. Records are not to be altered. Ms. Girgis must provide one week of original attendance douments which are written in ink. Submit documentation to OCCL by November 20, 2019.

Corrective Action: Ensure used disposable diapers are placed in a foot-activated container that is within arm’s reach of the changing area. At the time of the visit conducted on October 18, 2019, there was not a foot-activated trashcan in the diaper-changing area. This was corrected at the visit when Ms. Girgis brought a foot-activated container into the infant room.

Corrective Action: Ensure safe sleep practices are used at all times. At the time of the visit, one infant was observed sleeping in a swing. One infant was observed sleeping on the floor in a boppy pillow with a blanket covering his/her face. Two infants were observed sleeping in a bouncey seat. Two one-year-olds were observed sleeping on the floor with no mat/cot and no bottom covering. This was corrected at the visit when the infants were moved into cribs and the one-year-olds were moved onto cots with coverings.

Corrective Action: Ensure children's attendance is properly documented. At the time of the visit, six children were signed into the infant room, but eight children were present. This was corrected at the time of the visit when two children were signed into the infant room.

Corrective Action: Ms. Girgis does not model respectful communication when she yells at the staff. Multiple witnesses report observing Ms. Girgis yell at the staff in front of the children and parents. The administrator must model respectful communication interactions when relating the staff and children. The administrator shall ensure that yelling or other actions which may be frightening to children are prohibited. Ms. Girgis must complete the following two-hours of quality-assured trainings, which can be found on the Better Kid Care website: Embracing Professionalism Part 1: Where to Begin and Embracing Professionalism Part 2: Forward Visions. Submit training certificates to OCCL by July 22, 2019.

Corrective Action: Ensure proper staff-to-child ratios are maintained at all times. At the time of the visit, one staff was observed alone in a classroom with five one-year-olds and one infant. This was corrected at the visit when the infant was returned to the infant classroom with an additional staff member. Ensure children are properly supervised at all times. At the time of the visit, Child #1 was signed into the Cloud Room, but was not present in the classroom. Staff #2 was unaware of the location of Child #1. This was corrected when Child #1 was located in the Sunshine classroom and signed into the appropriate room.
