Noncompliance Area: At the time of the inspection, the water in Preschool 9 measured 111 F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: At the time of the inspection, the ceiling tile in the PS 9 class had water damage.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed child file #1, 2, and 4, who had all been in attendance for more than 60 days, did not contain an initial health report. Observed child file #3, a preschool child, contained a most recent health report which was dated more than 18 months ago.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: Observed there was no established medication log for 2 bottles of childrens pain relief in the toddler room.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: Observed child file #4 did not contain documentation of parental review of information in the emergency contact information and the financial agreement within the previous 6 months. Observed child file #4 did not contain signed parental consent for administration of minor first-aid procedures by facility staff.

Correction Required: Following review, a parent shall attest to the accuracy of information in the emergency contact information and the financial agreement at each review by affixing a dated signature to the record. A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Observed staff file #1, contained a volunteer child abuse clearance at the time of starting in a childcare position, which is not accepted for childcare employment purposes under the CPSL. Staff file #1 contains a current valid child abuse clearance at this time. Observed staff file #2, contained a volunteer State Police clearance at the time of starting in a child care position, which is not accepted for childcare employment purposes under the CPSL. Staff file #2 contains a current valid State Police clearance at this time.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: Observed in the infant/young toddler room, diaper creams labeled keep out of the reach of children, accessible to children due to a failed internal safety latch on the right side of the changing table.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
