| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance inspection, conducted on 7/22/2026, and are subject to change pending programmatic review. The fingerprint clearance cards for two of four staff members were verified as valid via the DPS website. The other cards were pending DPS decision. The plan of corrections is due within 10 days of receipt of this Statement of Deficiencies. NOTE: The license is pending correction of all deficiencies. The following was discussed, but not limited to: *Restroom exhaust fan operation, *Postings conspicuous to families at the entrance of the facility, *Classroom postings, *Electrical outlet coverage, *Restrooms maintained in a clean condition, *Playground equipment maintained in a clean condition, *Playgrounds maintained free from hazards, *Ensure children do not enter the brush area at the back of the grass field on the School-Age Playground, *Ensure children stay within the designated licensed areas on the playgrounds, *Ensure each activity area has all required toys, materials, and equipment for licensed ages. | |||
