The allegation report mentioned 3 leaky sinks, but the School Age Teacher stated that is not correct; there are actually 2 leaky sinks that are still loose and leaking. Both leaky sinks are located in the classrooms used by the children. Please arrange to have both loose and leaky sinks in the classrooms repaired so that they no longer leak and so that the basins are no longer loose. The staff explained that the multipurpose room did have a large hole in the dry wall, yet the hole was repaired back in February 2019. The LS observed a hole in the wall in Room 2 and the dry wall and column edge is damaged on a column in Room 2 as well . Please have the hole in the wall and the column in Room 2 repaired. Findings: The staff have confirmed that the center does have a cockroach infestation and to their knowledge, the center does not have a regular extermination company to treat the center. Please submit evidence that the center has made arrangements to have a contract for a pest extermination service to rectify the roach infestation. A metallic mouse trap is in the kitchen and an Orkin pest service sticker is on the wall of kitchen. Please ensure that the mouse box is checked regularly for trapped mice. Correct within 24 hours.
