Children were not adequately supervised at all times. A child was observed coming into the building from the playground to get a ball and was not accompanied by an adult.

A safe indoor and outdoor environment was not provided for the children. The following items were observed on the playground used by the preschool aged children: cracked boarder timber, cracked basketball goal, broken bike round-a-about, the bottom of the gate had a protrusion and rusted missing medal piece, and the paint on the wooden house structure was peeling. On the playground that the infants and toddlers use, there was a stake sticking out of the boarder timber.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two teachers did not have verification from an approved organization for CPR.

The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The asthma action plan on file for an albuterol inhaler was not current. The inhaler was located in the four-year-old room.

Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. There was an albuterol inhaler in the classroom of four-year-olds that had an expired prescription.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. The permission to administer medication for an albuterol inhaler was expired.

My 5 yr old son was left on All Smiles van on June 13 on an unauthorized trip to Blockbuster. The owner left him and never notified me. He was also forced to jump up and down in form of discipline and yelled at as well as other children. Please review www.ncchildcare.net and do research but I do not recommend this daycare.

My child truly loves her day care. She has really blossomed while under their care. I thank God for this establishment.
