Findings: The complaint alleges there are 30 children in the preschool room with one staff person during the opening and closing hours of the center. The licensing specialist arrived at the center at 8:15 AM. There were 18 children in the preschool room with 2 staff persons. The group included 2 toddlers, 2 two-year-old children, 4 three-year-old children, 2 four-year-old children and 8 school age children. This exceeds the maximum group size for a group with toddlers. The two toddlers were moved to the infant room and then another school age child and three-year-old arrived into the preschool room. Children continued to arrive and more staff arrived at 8:30 AM. The two-year-old room was not in ratio because one staff person had to man the front door and get some school age children on the school bus. The center's toddler teacher is out because she broke her foot. The center is hiring a school age staff person who will do a split shift and the school age children will then be in the upstairs classroom. This will decrease the capacity in the preschool room during opening hours and closing hours. The center is also interviewing for other positions.

Findings: The center utilizes face-to-name attendance sheets in all classrooms. Children are signed in and out; staff persons log in and out on the sheets as well. At the time of the incident, the face-to-name sheet was checked once the staff and children were inside of the movie theater instead of right outside of the van. Once inside the building, attendance was taken and the staff persons realized SA, age 3, was not with the group. The staff person went back out to the van and retrieved the child.

Findings: The center self-reported an incident in which a 3 year old child was left on the center van at the movie theater in Brandywine. It was reported that on Tuesday, June 28th, the child was left on the bus for no more than 2-3 minutes per the center staff persons. There were 14 children ( ages 3 through school age) and 2 staff persons. The group got inside the movie building and the staff realized a child was missing. The staff person went back out to the van to retrieve the child. The child's parent was notified of the incident immediately. According to the director, the mother was upset at the time and the staff person was also upset and very apologetic to the parent. The child was awake on the van and reportedly told her mother that they left her. The child is still enrolled at the center. The center has cancelled all off site field trips for children under the age of 5 and is instituting more on site activities/special visitors. A memo was sent out to all parents regarding the change in activities. After reviewing the planned off site activities, the center decided that some may not be age-appropriate for the preschool children.
