On 7/27/21 Staff #1 used a personal vehicle to transport two children to the facility, and not the authorized facility van.

The children waited for the facility to pick them up after school, because of a "delay" per facility management. Facility staff stated that the children were picked up late because "it is hard to have everyone picked up when they need to be picked up".

Findings: Per allegation and based on the Surveyors' observations, in the 2's classroom, staff in the room were not supervising the enrolled children in their care. When the Surveyors entered the room, an enrolled two-year-old child was observed standing on top of a table in the back of the room without staff knowledge. One staff member was changing an enrolled child and the other staff member was assisting an enrolled child in the restroom. Surveyor #2 removed the child from the table.

Findings: Based on the Surveyors' observations, in the 2's classroom, the staff member changed the enrolled child's pull-up by having them stand in the toilet room rather than placing them on the diaper changing table as required.
