A safe indoor and outdoor environment was not provided for the children. On the playground, two bird feeders were observed to be rusted.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In an unlocked adult restroom, a small canister of aerosol room spray and a stick of deodorant labeled Keep Out of Reach of Children with additional warnings was observed in a drawer. In Space 2B, a spray bottle of Screen Clean labeled Keep Out of Reach of Children with additional warnings was observed in an unlocked cabinet.

Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. The permission to transport form for one child enrolled was not available for review on the bus.

The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. A physical copy of the EPR plan had not been updated since 11/01/2016.

A violation was found for which there is no item number. Child Care Sanitation Rule 15A NCAC 18A .2825 WALLS AND CEILINGS (a) states that in child care centers, the walls and ceilings, including doors and windows, of all rooms and areas shall be kept clean, free of visible fungal growth, and in good repair. All walls and ceilings shall be easily cleanable and free of peeling, flaking, chalking, or otherwise deteriorating paint. In Space 2B, a small section of the wall near the base of the bulletin board near the cubbies, was observed to be peeling.

All equipment and furnishings were not in good repair. A broken plastic container sharp to the touch and a broken ball-launching tool were observed in the sand box on the playground.
