| A safe indoor and outdoor environment was not provided for the children. On the infant playground (furthest to the left when standing on the playground and facing away from the building, three vertical strips that frame the outside windows of the building were observed to be accessible to infants and toddlers and were broken and sharp to the touch. Two plastic gutter covers were observed to be broken, and one of these covers was sharp to the touch. Five push pins were observed to be stuck into the wooden siding of the infant playground, which present a potential safety hazard for infants and toddlers if they were to become loose. Two rusted nails were observed protruding from the wooden base of a set of windows. A metal bracket attached to the gutter closest to the front of the building (facing the parking lot), was observed to be broken and sharp to the touch. On the middle playground, a slim, vertical piece of wooden framing had come detached from the side of the building, and rusted nails were exposed. One plastic gutter cover was observed to be cracked and sharp to the touch. On the third playground used for older Pre-K children, a plastic storage cupboard was observed to have one rusted screw protruding from the bottom right corner.
