I have used this provider for less than 6 months

This daycare is dishonest and unprofessional. I provided 2 weeks discontinued service request per her agreement. I was told that I would get back a refund as she was going to back date the last day from the last time I brought my child. I still am unable to reach her to get the refund and my child’s belongs.

Findings: Observed no evidence of the provider notifying the OCC office of the incident on May 2018, of the child getting his hand closed in the door by another child. The provider stated that she called the mother immediately after the incident and the mother took the child to the doctor. The mother call the provider to inform the provider that she taken the child to the doctor, the child received a soft sling or wrap for hand and the child will return to care.

Findings: Complaint investigation conducted on today's date. Ashley stated that her back was to the children playing with each other and another child closed the child's hand in the door. This incident happen May 22, 2018. The provider called the parents immiadetely and the mother of the child pick up and took the child to the doctor. The child returned to care a few days later. Provider stated that the child's mother called and stated that the child received a soft sling and the child was returning to the daycare.
