The staff members named 19 children when identifying their primary care groups. Staff member 2 had 11 name to face cards and staff member 3 had 8 name to face cards. Per SIN-00285575, the providers correction required stated that the legal entity will be responsible for implementing a name to face card system to assist staff with supervision and primary care groups

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Directed Plan of Correction: 1. Facility agrees to unannounced visits by DHS representative for a period of two months in order to monitor supervision and regulatory compliance. The bottom of the desk in the young toddler classroom was falling apart and wood chippings were on the floor.
