Noncompliance Area: On 01/29/20, at approximately 3pm, an infant child was left to sleep behind a closed door in another room unsupervised.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Noncompliance Area: On 01/29/20 at approximately 3pm, agents of the department arrived at facility and observed at least 8 children, all visibly toddler age or younger, in care. Staff person #1 stated that 2 of the children were related to the operator, one infant and one toddler. Staff person #1 stated that 4 of the children present were infants and 4 of the children present were toddlers.

Correction Required: The operator may provide care to no more than five related and unrelated infants and toddlers at any one time. No more than two related and unrelated infants may receive care at any one time. If two infants are in care, three toddlers are permitted.

It was observed also in the outdoor play space the following toxic materials were left accessible to children under carport area: latex caulk, small propane tank, container of car oil, liquid plumber, windshield washer fluid, and a bucket of previous mixed paint; the large shed in the back yard was left accessible to children and contained multiple cleaning products and Tiki lamp fluid.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: It was observed that the window screens in the front room, where children receive care, contain holes and are not in good repair.

Noncompliance Area: It was observed that the first-aid kit used by facility was missing scissors and soap. It was observed that these items were later added to the first-aid kit the day of inspection.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: It was observed in the bathroom there were razors stored in the closet accessible to children. It was observed in the outdoor play space that there were multiple visible hazards accessible to children such as: crumbling and peeling shed wall, boards in fire pit containing rusted nails, rusted metal fire pit lid, picnic table falling apart with a board and sharp screw exposed, and children have direct access to the visible hazard of the heavily traveled main road in the front of facility from the play space through the car port and through the unfenced side of the yard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.
