Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/25/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 9 of 9 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Updating Children's Records with medication information. 2. The Statement of Services must include the 19 required items. 3. New rules were discussed. 4. Toilet seats must be secured. 5. The fire drill log with times of the drill and the time it took to evacuate.

Initial Comments: The following deficiency was observed at the time of the annual Compliance Inspection conducted on 8/27/2024 and is subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Self-Evaluation link was emailed to the facility director. The Emergency Disaster Contact form was completed at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Maintenance of outdoor plants, and Fire drill log scheduling. Compliance Officer #1 is Tricia Tartaglio Compliance Officer #2 is AuReyon Thompson.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 8/30/2023, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey was completed at the time of the inspection.
