Initial Comments: The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 4/10/26 and are subject to changes pending programmatic review. A full inspection was conducted. Please submit the Plan of Corrections via the LMS portal within ten days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was emailed to the Provider at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Lesson plans must be dated for the current week. *Infant feeding plans must be complete. *CBC results must be in each staff file. *Toxics, chemicals, and hand sanitizer must be inaccessible to enrolled children. *Outlets must be covered with a safety plug or insert. *Cords must not pose a tripping hazard. *The sticker on the diaper surface must be removed. *Faucets must be secured. *Peeling rubber on the playground must be secured. *Burr grass must be removed from the field if children use the field. *Refrigerators must be maintained at 41 degrees F or below. *Specific reasons for medications must be listed on the medication form. *Peeling paint in the toilet room.

Initial Comments: Amended 5/6/2025 to remove R9-5-516.A.4.a The following deficiencies were observed at the time of the Compliance Inspection conducted on 04.14.25 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the notice of the Inspections Rights and a Small Business Bill of Rights to the person in charge at the time of the inspection. Please submit the Plan of Correction via the LMS portal within 10 days of receipt. 7 of 7 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The following was discussed but was not limited to: 1. Enrolled children should not be mixed with non-enrolled children. 2. The current license must be posted. 3. Thorns must be inaccessible to enrolled children. 4. Soiled clothing containers must have a lid. 5. Closed containers in toilet rooms. 6. Field trip forms must have the description of the field trip. 7. Medication forms must be complete with specific symptoms to watch for to administer medications. 8. Resilient surfacing below the stairs of the loft. 9. The stickers on the diaper surfaces must be removed. 11. The Statement of Services must include emergency medical procedures.

Initial Comments: The following deficiency was found at the time of the Compliance Inspection conducted on 4/19/2024 and is subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but not limited to: 1) Obtaining Staff 1's and Staff 4's DCS clearance results, 2) Submitting the City of Phoenix Fire inspection when obtained, 3) Reminder of the Anniversary fee due in July 2024, 4) Crib spacing requirements when cribs are occupied, and 5) Tummy time rules and requirements. There were 5 staff files reviewed. 5 of the 5 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
