Description: Child 5 and Child 6, as listed on the child record checklist, received all doses of all immunizations on the same date. Child 5 received all immunizations on 8/30/3010, and Child 6 received all immunizations on 4/26/2017. On 12/27/2017 the center requested the correct immunization dates by email, but none were in the children's files yet.

Description: The Youth Development Director of School Age Child Care reported that on 9/19/17 a YMCA staff member was not present to be responsible for and supervise children who arrived at the center between 7:00-7:35 a.m. for the before school program. A school staff person supervised the children beginning at 7:20 a.m. when they were discovered without supervision. It was learned that on 9/12/17 a YMCA staff member was not present at the before school program between 7:00-8:00 a.m. as well. A school gym teacher supervised the children for at least part of that time.

Description: The immunization histories maintained by the licensee for 3 children (Child 12, Child 13, and Child 14 at the top of the child record checklist) did not include accurate immunization dates. Immunization histories for all 3 children document that all doses of all immunizations were received on 1/19/2001, prior to their births.

Description: The refrigerator was 70 degrees F. when it was checked; the refrigerator had been left partially open. I closed the refrigerator all the way and went back at the end of the monitoring visit to check the temperature. The door was left partially open again; the refrigerator was 70 degrees F.

Description: A medication to be administered to Child 9 (on the child record checklist) if there was a serious allergic reaction, had expired in April 2017.

Description: The medication authorization for Child 11, on the child record checklist, did not include the length of the authorization (no beginning or ending dates). A medication for Child 10, on the child record checklist, had authorization from the doctor, but not from the parent.

Description: The licensee has an approved exception to keep staff files at another location with the condition that emergency information for all staff that work at the center must be on the premises of the center. One staff person was missing emergency information on site.
