Description: The ratio of child care workers to children was less than the minimum number of child care workers to children specified in Table 251.055 on 1/17/2020 when 13 children ages 1-2 were with 2 teachers from 8:11 AM-2:50 PM and there were 17 children ages 2-5 years in the EPS/PK room from 11:00 AM-3:40 PM. On 1/24/2020 there were 19 children ages 2.5-9 were with 1 teacher from 10:30 AM-1:00 PM.

Description: An employee at the center had reasonable cause to suspect that a child had been abused as defined in ss 48.02(1) and 48.981(1) Stats., and did not immediately contact the county department of social services or human services or a local law enforcement agency, as required by s. 48.981, Stats. when, according to staff interviews, a staff member suspected that a child had been abused by a staff member on 12/9/2019 and she did not contact the appropriate authorities.

Description: According to staff interviews, a child care worker used an action that may have been cruel, aversive, and humiliating on December 9, 2019, when the child care worker used a loud tone of voice while she abruptly picked a 3-year-old child up by the biceps, lifted him several feet and forcefully put the child down to correct his behaviors. The child showed signs of being upset and frightened after the correction. The child care worker stated that she knows her voice is inappropriate but was not aware that she could not use physical force to correct behaviors of children in her care.

Description: The licensee did not report to the Department any prohibited actions specified in s. DCF 251.07 (2) (e) by an employee or volunteer to a child in care, within 24 hours after the incident when an incident occurred on 12/9/2019 and it was not reported to licensing.

Description: The regularly assigned child care teacher and assistant child care teacher for each group of infants and toddlers did not have a minimum of 10 hours of training in infant and toddler care approved by the department within 6 months after assuming the position when the Staff A, regularly assigned to the toddler room, did not have documentation of the required training after 6 months of working in her position.

Description: A staff member used actions that are cruel, aversive, frightening and humiliating when other staff reported witnessing a staff person grabbing, children by the arms, yanking, pulling and yelling at children and forcing children on their cots.

Description: Staff B did not have documentation of a physical examination report on a form provided by the department that was completed not more than 12 months prior to nor more than 30 days after Staff B was hired and indicating that Staff B is free from illnesses detrimental to children, including tuberculosis, and that Staff B is physically able to work with young children.
