| Initial Comments: There were no deficiencies found at the time of the Initial Inspection conducted on August 14, 2026, and are subject to changes pending programmatic review. There were four personnel files. All four of the fingerprint clearance cards were verified to be valid through the DPS website at the time of the inspection. A copy of the Notice of Inspection Rights was provided to the group home at the time of inspection. BCCL staff will email the Empower Self-Evaluation Assessment link to the Provider.
