|Findings: Based on review of documents provided, as well as Director statements, the Team Leader noted that a current gas inspection report was not available. The most recent inspection available was dated 12/18/2015. *The facility scanned a current gas inspection to the Department on 01/20/2017*|
|Findings: Based on review of required inspection reports, the Team Leader noted the following: -The facility presented a fire inspection dated 06/09/2016, but was not able to provide documentation that the necessary repairs/corrections listed on the fire inspection report had been completed.|
|Findings: Based on review of staff files, the Team Leader noted that Staff #1's file did not contain documentation of 18 or more hours of annual training. There were only eight training hours documented for the 12 month period of 01/31/2015 to 01/31/2016.|
