Description: Learning, Enrichment, And Play (L.E.A.P) programs are licensed facilities available year-round throughout the Marana School District. This program is designed for students enrolled in kindergarten through sixth grade, and is offered Monday-Friday, before and after school, from 6:00 am to the start of school day, then from school end until 6:00 pm, during the school year.

Findings: The Team reviewed the children's Emergency, Information, and Immunization record cards and found that Child #2's card did not have an immunization record attached. The Director was able to locate a copy of the immunization record and attached it to the card during the time of the inspection.

Findings: The Team Leader reviewed the roster used to document the presence of enrolled children and found that there were 9 children present and 6 children documented on the roster. The roster was not up-to-date.

Findings: The Team Leader asked to review the staff file for Staff #1 and found that the complete file was located in their district office and not on-site. The file was scanned to the facility during the inspection and able to be reviewed during that time.

Findings: The Team Leader observed an unlocked cabinet in the restroom used by enrolled children. The cabinet contained cleaning chemicals and disinfectant wipes were locate on top the cabinet. The Team Leader informed that Director that because school aged children use the restroom, the items would need to keep the cabinet locked to ensure they were inaccessible to them.

Findings: The Team Leader reviewed the lesson plans and found that they needed more information to be documented. The lesson plan for April 8th was "feather painting" and April 9th's lesson plan was "tissue paper dragon fly", etc.

Findings: According to the Director, the medications are kept in an unlocked file cabinet located in the Director's unlocked office. Therefore, the medications are not being stored in a locked cabinet or container.

Findings: The Team Leader observed a grate that was designed to cover a hole in the ground on the outdoor activity area. Approximately half the grate was no longer attached to the wrought iron frame or fencing wrought iron bars located throughout the structure. The structure was located next to the building and was at least 40 inches long. The children's feet would fit through the gaps. According to the Director, he wasn't allowing children to play in that area so that they wouldn't get hurt.
