Resolution: The provider has implemented new supervision strategies such as separated age groups and increased the amount of staff supervising the children. Additionally, the staff on duty during the incident are watching a different age group of children. Lastly, biting toys are supplied and sanitized daily.

Findings: There was a supervision concern of a child being left unattended. During an investigation, it was determined that a child was left unattended on the playground. Resolution: The provider issued a written warning to the staff member as a disciplinary action and the staff member has completed supervision trainings. The provider has implemented new supervision protocols.

Findings: Serious Injury. There was a concern that a child sustained a bone fracture while in care. During an investigation, it was confirmed that an injury did occur when a child who was playing on a toy, jumped off and sustained a fracture. Resolution: The child care provider reported the incident appropriately. The child received medical attention for the accident and the matter has been resolved.
