Findings: Serious Injury. The provider self-reported that a child had fallen off the slide and taken to urgent care by mom for a possible broken arm. During the investigation, the health department determined the child stood on the slide and tipped landing on their arm. The child's parents were called and mom came and took the child to urgent care confirming a broken arm. Resolution: The staff member supervising the children at the time was let go. Additionally, the owner plans on adding additional soft ground material to the playground area for safety and children are no longer permitted to stand on the slide.
