Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 2/26/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Staff files must be maintained in a single location on facility premises. *Discussed hosting a training for the staff before the next school year. *Discussed having a one-on-one training with the Director.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 03/05/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contact Group Size was in compliance at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *On the Emergency Information & Immunization Record Card the two emergency contact names and phone numbers can not be mother or father. *The garden boxes located on the South end of the playground need to be repaired. *Monitor the green rubber resiliency surfacing.
