| Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 2/5/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The following was discussed but not limited to: 1) New rules effective 8/3/2025, 2) All current staff are required to apply for DCS clearance through the DCS portal that was effective 7/2024. The DCS portal tutorial link was sent to the licensee, 3) Snack menu is required to be posted in same posting area location as other required postings, as listed in R9-5-303 2, and 4)Temporary absences on a roster are required to be recorded in a permanent method and the roster must be retained for 1 year. Currently, the licensee is recording movement on a dry-erase sheet that is erased at the end of the day. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | |||

The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Criminal history affidavits are 2 pages. 2. The room capacity must be posted in each activity area. 3. Children's attendance rosters must be kept for 12 months. (Dry erase boards cannot be kept since they are reused daily.) 4. Refrigerator temperatures must be maintained 41 degrees F or below. 5. Children should wear shoes at all times. 6. Remove the broken pink rocker on top of the fence. 7. All staff must record times of arrival and departure. 8.
