Outdoor play area has tall grass and weeds present, up to approximately two feet tall in some areas. Four rooms accessible had large amounts of clutter and large piles of laundry on the floor.

Various hazardous items were accessible to children. A bucket containing a large amount of cigarette butts was present in the outdoor play area. A prescription medication was present in a room accessible to children. An electric razor was plugged in on the counter in the bathroom used by children. Hydrogen peroxide and various toiletries were accessible on the bathroom counter.

Floors were not clean and sanitary in some areas used by children; various types of small debris present throughout.

The home is not clean and is not in sanitary condition. Licensing observed debris covering carpeted areas in the living room, and in the children's playroom. There was also debris, and food waste on the floor in the kitchen. There was pet waste in three different areas of the home at the time of the visit. The attic access was open in the back bed room as well, this room is accessible to children.
