Primary Caregiver's PDL expired 12/5/2024. Upon arrival there were 4 infants and only 3 playpens set up. Upon review of licensing one play pen mattress pad board was found to be broken. Meaning that only 2 playpens were available for use on this date. One child was missing the first page of the Child information form being used by the program. One child was missing the immunization record. The living room and kitchen were cluttered with bagged food on the floor of the kitchen, laundry on the couch, and surfaces covered with paper, books, and other items. Primary Caregiver and Assistant caregiver on-site and counting in ratio did not have current CPR and First Aid training. Expired on 12/28/2024. |
