A safe indoor and outdoor environment was not provided for the children. Today boxes and cots were stacked in the hallway outside room #5. These items were moved to rm 6 today. Rm 6 was used for school age children however at this time there are 2-3 school age children in care, and they join the preschool children in room 5 in the afternoon. Currently room 6 is being used for storage. There were metal folding chairs, childrens chair, folding tables, cots, and other materials stacked on the covered patio area of the playground.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. WR a substitute working in a classroom today did not have an Emergency Information form on file.
