| Findings: Health & Safety Concerns. There were concerns of the provider's ability to keep up on health & safety standards including clutter, diapering practices, fire code requirements, and outside and inside play area safety standards. During an investigation, there was a diapering pad that was ripped, chemicals accessible to the children, excessive clutter & debris in outside play area, and no emergency access in the "sleeping room". Resolution: The provider added proper emergency access in the sleeping room, the diaper pad was covered with a back-up on site, and the outdoor play areas were clean-up and safe for children. | |||

| Findings: Health & Safety Concerns. There were concerns that there were trash, toys, and all sorts of clutter covering the floors and counters. Additionally it was noted that there was dried throw up and garbage in the infant sleep room. During the investigation, these allegations were confirmed. Additionally, multiple playpens had large holes in the mesh causing a strangulation hazard. Resolution: The yard had been cleared of hazardous debris by provider's family. Playpens that were torn were removed from the premises. The facility was mostly free of clutter and safe for children to be in. | |||
