Partnering with parents and utilizing a qualified, professional faculty and staff, UGCS trains young Christian men and women for a lifetime of service and provides the academic knowledge necessary for entrance to college through Christ-centered, Biblically-based classes.

A safe indoor and outdoor environment was not provided for the children. Today two swings on the playground outside did not have their S hooks closed.

A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. All children's files that I reviewed today did not have this form in their files.

Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. This form was not present in all the children's files I reviewed today.

The depth of the loose surfacing was not based on critical height of the equipment. The mulch measured 2.5 inches deep around two very large pieces of equipment.

A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. This form was not in all of the children's files.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff had an expired First Aid training.

Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last recorded practice was on March 16, 2023.

The depth of the loose surfacing was not based on critical height of the equipment. The swings did not have enough mulch under them to be in compliance with the height of the swings. Mulch must be two times the pivot point.
