Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence area to the back right corner measured less than 4 ft. high.

A drug or medicine was administered after its expiration date. Several diaper creams and sunscreens permission forms were out of date and being administered to the children.
