Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 12-17-2025, and are subject to changes pending programmatic review. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group Size was compliant at the time of the inspection. The following was discussed, but not limited to: 1. Posted lesson plans. 2. Discussion about diapering in toilet rooms. 3. Uncovered outlets. 4. Items unrelated to diapering in the diaper-changing area(s). 5. Thermometers in fridges/freezers. 6. Diaper-changing sink temperatures. 7. Central Registry background check requirements. 8. Criminal History Affidavit and fingerprint clearance card renewal requirements.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 12/19/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DES website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Make sure the resilient surfaces (woodchips) are located in the fall zone areas. Initial Comments: The purpose of the inspection was to conduct a Monitoring Inspection for Complaint #0062607. A full inspection was not conducted at this time. There were 0 staff files reviewed during this investigation due to record retention policy.
