BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 5 of 5 fingerprint clearance cards were verified as valid via the DPS website during the inspection. The following was discussed, but not limited to: *Posted lesson plans to reflect the current date. *Statement of Services to include all requirements. *All designated exits must remain unlocked and unobstructed during business hours. *Tighten loose toilet seats.

Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 04.16.25 and are subject to changes pending programmatic review. Initial Comments: The purpose of the inspection was to conduct the complaint case number# AZ00061440 investigation on 7/25/2023. A full inspection was not conducted at this time. Compliance Officer #1: David Ramos Compliance Officer #2: Stephanie Jake Ratios observed were: infants-2's: 2:5 2's: 1:8 3's-5's: 2:12 School-age: 1:10 There were 3 staff members interviewed during this investigation. Others interviewed: complainant by phone on 7/20/2023 but no contact was made. Documentation observed were: class rosters, class rosters for the month of June 2023, and attendance records for the month of June 2023. Upon completion of complaint case # 00061440, it was determined from observations, documentation, and interviews that the allegation was substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections through the Department's online portal within 10 days of receipt of the Inspection report.
