Findings: There was no barrier placed at the bottom of the stairs leading to the main level of the home. The barrier was there, but slid to the side and not in place as a barrier. Provider reported that she no longer uses it after placing a barrier at the entrance to the play area. However, children were observed moving that barrier by themselves and coming into the additional day care area that includes the entrance to the stairs. The children also have to go into that area to go to the bathroom. Therefore, the barrier at the bottom of the stairs needs to be put back in place and remain in place. Please show picture evidence of having the barrier to the stairs in place.

Findings: Two year and three month old, JB, was placed in a crib for nap time. He was also observed to be standing in it and rocking it. This is a safety hazard as it seemed he could easily tip it over with him in it. The regulation states that a child 12 months old and younger than 5 years old shall be provided a bed, cot, mat or sleeping bag. Please provide a letter stating how you will become in compliance with this regulation in the future.

Findings: Upon further inspection, both cribs present and being used in the daycare do not have any brand label or model number in order to determine whether or not they are appropriate or if they have been recalled. They also present as unstable. The Provider had adjusted the bottoms (seemingly plywood) up and down without any predetermined holes. Subsequently, these cribs cannot be approved as appropriate sleep sources. Provider has agreed to stop using these cribs. Provider will be limited from having infants until appropriate sleep sources can be purchased and approved.

Findings: During the inspection, the two cribs were observed to have thick blankets in them. One of the cribs also contained a boppy. Provider stated that the parents brought those items and also use them at home. Discussed concerns with suffocation hazards with Provider. Nothing should be in the crib other than a snuggly fitted sheet on the mattress. Please remove these items and notify the respective parents of the suffocation hazard. Write a letter stating how you will become in compliance with this regulation in the future.

Provider stated that she had it on because of the inspection. Licensing Specialist informed Provider that she was to go about her program as if I was not there and Licensing Specialists are to observe the Program. Provider was reminded of the use of technology limited to only thirty minutes per week. Please write a letter of compliance stating how you will become in compliance with this regulation in the future.
