Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Children were not signed in for 12/21/23.

Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. I observed shattered glass in between trees, beside the shed, and near the deck. I observed trash on the deck that consisted of a large box, plastic bags with diapering materials and a drawer tower. I observed debris beside the deck along with large black lawn bags near the shed/fence area. I observed broken carpentry equipment. There were wooden pieces of the fence by the play structure and near the fence areas.

When screen time was provided, it was not offered to stimulate a developmental domain in accordance with NC Foundations for Early Learning and Development. A school-age child was watching YouTube videos on a phone in which did not meet any of the NCFELD domains.

All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. I observed the following items in the outdoor learning environment 1) fence door missing 2) the fence had an area near the steps with broken and deteriorated wood and protruding nails 3) the outdoor shed was not locked 4) parts of the fence on the backside is learning forward 5)broken glass was observed all around the outdoor area 6) several branches and tree debris was observed. This is a repeat violation.

An individual living in the household who had their 16th birthday after the initial licensing of a family child care home, did not complete and submit required forms within five business days. S.F did not have a CBC letter on file and available for review. This is a repeat violation.

Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The provider has not completed the health and safety trainings. This is a repeat violation.

An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Five (5) children were sleeping on the floor without a cot or mat.
