A safe indoor and outdoor environment was not provided for the children. On the playground used by School Age children, the plastic base to a metal gutter located on the side of the building closest to the neighboring elementary school, was observed to be cracked and sharp to the touch. One outlet in Space 1 and three outlets in Space 15 were observed without safety covers; each of these was corrected during the visit. Plastic bags were observed to be stored accessible to children under two years of age inside cubbies in the following spaces: Space 5, Space 7, Space 8, and Space 11; all plastic bags were made inaccessible to the children during the visit.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One spray bottle of Clorox cleaning spray was observed in an unlocked bathroom cabinet shared by Spaces 7 and 8; the key was still observed in the lock.

The center did not maintain records as required in rule, and/or were not made available to the Division for review. A new file for one staff member, who ceased employment and then started employment again greater than 12 months later, was not created upon return to work.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Benadryl, prescribed as part of a medical action plan, was observed in unlocked storage in Space 8.

Parent's medication authorization did not include required information. The diaper cream forms for greater than twenty children enrolled across the classrooms for infants, toddlers, and two-year-olds did not have a specific dosage amount recorded or a specific time documented to apply the medication.

Medical exam or health assessment record was not on file before or within 30 days after admission. The medical reports for two children enrolled were completed only by the parent/guardian not a physician.

The EPR Plan did not include the location of the Ready to Go File and or the required information. The contents of the Ready To Go file included an allergy list from 2021 and employee emergency information sheet/health questionnaires from 2021. The current and required information was not in the Ready to Go File.

When I was about 4 I went to today’s child. There I meat my friends. They are Brennan, Trin, Noah, Saywer, and Holden. To all the staff that remembers me I miss you guys stay safe and be positive. ????
