Initial Comments: There were no deficiencies observed at the time of the initial monitoring inspection conducted on 9/11/2025 and is subject to change pending programmatic review. A full inspection was not conducted. The following was discussed: minimizing the unevenness between the rubber mats on the outdoor activity area to minimize tripping hazard.

Initial Comments: The following deficiencies were observed at the time of the initial compliance inspection conducted on 7/31/2025 and are subject to change pending programmatic review. The Compliance Officer reviewed 2 staff files. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed: not using one toilet room until the exhaust fan is installed, hot water heater enclosed, and the plungers removed, clarifying that the medication permission form needs to be completed before administering medications in the statement of services.
