| Initial Comments: The following deficiencies were observed at the Initial inspection conducted on 7/21/2026, and are subject to change pending programromatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficienies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 10 of 10 fingerprint clearance cards were verified as valid via the DPS website at the time of the inspection. The following was discussed, but not limited to: *Ensure broken classroom furniture is repaired, replaced, or disposed of. *Ensure electrical boxes in classrooms remain closed at all times. *Ensure hazardous items, such as exposed pencil sharpeners, are inaccessible to children. *Ensure supervision is maintained at all times in blind spots of classrooms. *Ensure diaper changing procedures are followed accordingly, including using a seamless and smooth surface and the soiled diaper bin is inaccessible to children. *Ensure toilet seats are tightened. *Ensure air vents are cleaned regularly and are free of dust buildup. | |||
