| Findings: There were supervision concerns regarding a child who left the facility. During the investigation, it was verified that a child was sent outside alone to get a blanket and then left the building through a gate that was not latched all the way. Resolution: The facility is not allowing children to leave the room unattended now. They are activating a button that is about 5 feet up that needs to be pressed in order to exit. The facility is figuring out a way to keep the gates latched at all times (carabiners). They are taking proper cautionary measures to ensure this does not happen again. | |||
