| Findings: Supervision Concerns. There were concerns that a child was by himself on the playground unsupervised. During the investigation, it was confirmed that a child was able to go missing from a staff member's class for a significant amount of time. Resolution: The staff member was dismissed and the parents were notified of the incident. At the time of inspection, all children were properly supervised and a follow-up inspection will be conducted to ensure adequate supervision is maintained. DUring follow-up inspection, name tags had been implemented to prevent reoccurrence. | |||
