The program held a staff meeting and demonstrated how to use the straps on all high chairs. The teacher that did not properly report the accident was terminated. |

Plan: The program has educated all personnel on how long a child can be in restricted equipment. The program has placed timers behind highchairs. As soon as a child is strapped in, the timer is started and it counts down from 20 minutes. The "beep" on the timer is loud enough that the kitchen staff and front desk staff can hear and check on the child to ensure compliance. |
