I have used this provider for more than 6 months

I would not recommend this place, as my child was physically assaulted by an employee at this establishment. I have learned that this was not the first time a child was physically assaulted at this establishment. I did press charges.

Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two (2) infant bottles were not dated and labeled.

When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. A child with prescription diaper cream did not have documentation of applying the medication each time it was used. Instructions indicated to apply after each diaper change. Documentation was noted on 12/1/23. The child was absent on 12/4/23 but was present 12/5/23 and no documentation of applying the medication was noted on 12/5/23.

Staff did not interact with children in positive ways by helping them feel welcome and comfortable, treating them with respect, listening to what they say, responding to them with acceptance and appreciation, and/or participating in activities with the children. A teacher in Space 7 was heard from the hallway directing children to clean up. She was heard raising her voice and stating "pick it up!" to a child. When I walked into the classroom the teacher was observed sitting on a chair organizing materials in bins. Her back was to the door and she was heard and observed using an inappropriate tone and inappropriate volume as she told children to clean up. The children were across the room on a carpet.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The safe sleep chart was completed accurately. Four (4) safe sleep checks were documented after the child woke up from nap according to the iPad.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On April 19, 2023, nine children, age two, were present and witnessed a staff member discipline a child using rough handling.

Child was handled roughly. On April 19, 2023,a staff member disciplined a two year old child by roughly pushing the child to floor on more than on occasion. The same staff member also swatted towards the child's leg.
