Explanation of violations: A 2 year old unfastened his safety strap himself while having snack. I took a child with me (with parental consent) to pick up children at school and forgot to sign us out when we left. A child's medical form (standard enrollment paperwork) was accidentally placed in between two folders and found later that day.
