The Children's Center of Tucson, AZ offers an Early Childhood Development program for children newborn to 6 years of age. The Center's purpose is to provide a rich environment, which will enhance and stimulate the total development of the child, physically, emotionally, socially, and intellectually.

We offer creative learning activities which incorporate reading readiness, math readiness, and arts and crafts. The educational materials in the environment are also designed to provide a wide variety of learning experiences, geared to the developmental needs of each child. These experiences will allow the child to have positive feelings about his or herself and build capabilities in all the following areas: Physical Development, Emotional Development, Social Development, and intellectual Growth.

The Children's Center staff is carefully screened and selected for experience and commitment to young children.

Initial Comments: The purpose of the inspection was to conduct a Complaint Investigation for Case #166931 on April 23, 2026. A full inspection was not conducted at this time. The following classroom ratios were observed: *1:4 Infants *2:8 Ones *1:8 Twos *2:16 Three/Four/Fives There were six staff members interviewed onsite during this complaint investigation. There was one Fingerprint Clearance Card verified on the DPS website during the investigation. Documentation observed included Classroom Rosters, one Emergency, Information and Immunization Record (EIIR) card, one staff file, video footage of the alleged incident, child sign in and out documents, staff sign in and out documents, and the facility Statement of Services document. Upon completion of the complaint investigation it was determined from observation, documentation and interview that the allegations on Case #166931 were not able to be substantiated at this time. Items discussed, but not limited to, were as follows: -Mandated Reporting guidelines and procedures -Incident report requirements -Roster documentation on playground, in classrooms, etc. -Updated process DCS Background Check process -New forms and rule set available on AZDHS website There were no deficiencies observed at the time of the Complaint Investigation, subject to changes pending programmatic review.

Initial Comments: The following deficiencies were found at the time of the annual Compliance Inspection conducted on 06/16/25, and are subject to changes pending programmatic review. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 02/27/26 Fire: 05/21/26 (Tucson Fire Dept.) Gas: 06/17/26 Sanitation: 11/30/25 Items discussed, but not limited to, were as follows: -Updated DCS Background Check process -Fire extinguishers due by end of June 2025 -Transfer sleeping infant to crib -Allergy/modified diet posting must specify items -Add N/A or line through second parent/guardian on EIIR Please complete the Plan of Correction via the online Portal within 10 days of receipt of this Statement of Deficiencies.

Initial Comments: The following deficiencies were found at the time of the annual Compliance Inspection conducted on June 18, 2024, subject to changes pending programmatic review. A full inspection was conducted at this time. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 02/27/25 Fire: 05/21/25 Gas: 06/17/25 Sanitation: 11/30/24 Items discussed, but not limited to: -Updated DES/DCS Background Check process -Specify flavors served on menu (yogurt, muffin, etc.) -Keep designated exit pathways clear -Fence gaps must be 4 inches or less (vertical & horizontal) -Update Infant Feeding Instructions as needed -Put N/A on child emergency card if second parent/guardian is not listed -Updated Entrance Letter -Forms and rule set available on AZDHS website -Licenses are now valid for one year The Plan of Correction was not accepted at the time of the inspection.
